We hope that you will discuss any complaints or issues about your mental health services with your Service Provider Coordinator. You may file a grievance by talking to your Service Provider Coordinator or any other Mental Health staff. You do not have to complete a Grievance Form yourself, you may ask any Mental Health employee to fill this form out for you.
